Caring Companions, Care Team Weekly Rhythm
Office phones are covered every minute of 8am to 5pm, Monday to Friday, and a Lead Caregiver carries the phone every hour outside that. Nobody is ever uncovered. Field time never leaves the office empty. Hours: Krystal 8a–2p daily · Angiel Mon 10a–5p, Tue–Fri 1p–5p · Samantha ~10/11a–4p.

After hours & weekends — Lead Caregiver on call
Weeknight on call
Monday to Friday · 5:00pm – 8:00am
- Picks up at 5pm as the office closes, hands back at 8am when Krystal starts.
- Friday's shift runs 5pm to Saturday 8am, then the weekend post takes over.
- Stipend: $25 per night. Five nights Monday to Friday, so $125 for a full week of them.
Weekend on call
Saturday 8:00am – Monday 8:00am
48 hours, one person, one handoff
- Starts Saturday 8am and runs straight through to Monday 8am.
- Covers both weekend days and Sunday night, so Monday opens with no backlog of unanswered calls.
- Stipend: $100 per weekend, covering all 48 hours.
The two posts are filled separately. Somebody can hold weeknights without holding weekends, and the
weekend is its own stipend because it is a different commitment: two full days, not an evening by the phone.
Between the office day, the weeknight post and the weekend post, every hour of the week is somebody's, and the
grid above shows whose.

Krystal — Private Pay & LTC book

- Her clients: every Private Pay, LTC-insurance, and PACE lead, client, and family. The hub auto-assigns them to her.
- Owns the morning: overnight queue, lead callbacks, first-shift Care Match calls

- Field route Tue/Thu: private-pay referral sources (discharge planners, home health, assisted living, physician offices), logged in Referrers same day
Angiel — Medicaid, VA & CDS book
Mon 10am–5pm · Tue–Fri 1pm–5pm · field Wed & Fri 2–4
- Her clients: every Medicaid IHS and VA lead and client. The hub auto-assigns them to her.
- CDS intakes are hers too: warm-transferred when she's in, saved to her book for callback when she's not. The intake itself runs on the CDS side and rides the CDS hub's tracker.
- The monthly client check-ins are hers: the Client Check-ins board, Due This Week and Overdue at zero
- The State Submissions board is hers: every DSDS referral chased weekly until authorized
- Afternoon rhythm: follow-ups due, New Clients scheduling checklists, Needs Attention at zero by 5
- Field route Wed/Fri: Medicaid & VA sources (DSDS case managers, veteran service officers, senior centers), logged same day

- Wed & Fri 2–4: phone coverage while Angiel is out (exception handling, not reception, CEO work continues)
- Owner work: approvals, escalations, growth
The one rule that makes this work: the board is the handoff. Nobody briefs anybody at shift change. Every record shows its stage, its notes, and its next follow-up date in the hub, so whoever is on works whatever is sitting in the queues. Ownership is a label, never a wall, and everyone sees everything. In practice: your book is yours to drive, but whoever is on shift answers for every client while they're on, a Private Pay family calling at 3pm gets helped by Angiel, then Krystal picks it up from the notes tomorrow.
Growth plan: when the Medicaid and VA books get big enough, they split, one coordinator for each. The payer-based structure is built for that day: the map just gains a name.
Lead Caregiver — on call
Weeknights 5p–8a · Weekends Sat 8a–Mon 8a
- Answers the on-call phone: call-offs, no-shows, client and caregiver emergencies.
- Fills what can be filled from the caregiver list; logs what cannot so the office picks it up at 8am.
- Escalates to Samantha only for what genuinely cannot wait until the morning.
- Leaves a note on every call so nobody is briefed at handoff, the board is the handoff.
The field expectation, in writing
We get busy and the marketing is the first thing that gets skipped. So the field blocks are not a goal, they are part of the job, the same as answering the phone:
- Two field blocks each, every week, at the set times. They are appointments with the business. Treat them like client visits.
- Only Samantha can cancel a block, and a canceled block gets rescheduled inside the same week, never skipped. "It got busy" moves a block, it does not delete it.
- Every visit is logged in the hub's Referrers tab before the shift ends. A visit that is not logged did not happen.
- The scoreboard keeps the score, not memory. The Referrers page shows each person's count against 2, all week. From Wednesday on, anyone behind pace shows up red on Samantha's screen automatically.
- Monday's huddle opens with last week's numbers, out loud. One short week is life. A pattern is a conversation.
